Subject: Oxygen Cylinder Investigation
Dear Colleagues
As you are probably aware Qantas suffered a depressurisation event on one of our flights out of Hong Kong recently and immediately diverted to Manila. The preliminary report has now been released by the Australian Transport Safety Bureau (ATSB) .

Their initial findings have identified a failure of the number 4 passenger oxygen cylinder in the forward cargo hold of the 747. Unfortunately as written in their report no part of the oxygen cylinder could be located during the inspection of the aircraft.
This is a very rare event and as no cylinder evidence can be located, in the interests of Safety, I seek your feedback if you have any knowledge or experience with oxygen cylinder failures.
Your feedback would be of benefit to the investigation.
