Hot shutdowns led to CS100 engine-test explosion: inquiry

  • 06 JULY, 2016
  • BY: DAVID KAMINSKI-MORROW

Canadian investigators have traced an explosion and uncontained engine failure on a Bombardier CS100, during ground tests, to an oil-feed tube seal failure, resulting from misinterpretation of shutdown procedures.

Failure of the seal enabled engine oil to combine with the cooling airflow in the Pratt & Whitney PW1500G engine, and this air-oil mixture auto-ignited as a result of high ambient temperature.

The fire resulted in the failure of the whole turbine rotor stage, causing severe damage to the powerplant as well as the nacelle and the aircraft’s wing.

None of the six crew members on board was injured in the 29 May 2014 accident, which occurred during pre-certification work on the FTV-1 test aircraft at Montreal’s Mirabel airport.

Canada’s Transportation Safety Board states that the seal failed as a consequence of “insufficient cooldown” – the powerplant had been shut down after a high-thrust operation but without allowing enough time for internal temperatures to fall.

Pratt & Whitney had issued special instructions outlining cooling procedures before shutdown, with alternate solutions for hot shutdowns. The engine manufacturer had "not foreseen" the detrimental effect of "heat soaking" during hot shutdowns on the seals, the inquiry says.

“Bombardier interpreted the alternate shutdowns…as an alternative equal to the other shutdown options,” states the inquiry. “This resulted in the engine being exposed to one or more hot shutdowns, which led to heat-soaking beyond the design criteria of the bearing oil-feed tube’s seal.”

The engine testing on the day had involved two phases, beginning with the right-hand engine being run at various power settings to obtain cabin air samples.

This was to be followed by a left-hand engine run, to check for leaks on an oil-pump assembly which had been replaced the day before. The left engine would initially be run with the thrust-reverser doors open, before being shut down, whereupon the reverser doors would be closed and the engine restarted for a second run.

Running of the right-hand engine proceeded uneventfully and testing moved to the left-hand engine.

Investigators state that the engine was run at idle power, with no leaks found, before the reverser doors were closed and the engine restarted.

It was powered up to 60% of N1 for just over 15min, then thrust was increased to 74% for 5min 45s, at which point the explosion occurred.

The crew included two pilots in the cockpit. There was no fire-warning indication but the aircraft did generate engine vibration and failure messages on the engine indication and crew alerting system screen.

Investigators state that the commanding pilot secured the left-hand engine and shut down the right-hand engine, but did not activate the fire bottles. The commander ordered an evacuation of the jet while the second pilot requested airport firefighting assistance.

Initial efforts to extinguish the fire, by Bombardier personnel, took place about 2min 30s after the explosion.

Airport firefighters arrived some 2min later, and the first fire-suppressant attack from emergency vehicles commencing 3min after that. The fire was declared extinguished 13min after it began.

Investigators state that, although ground personnel successfully dealt with the fire, there had been an “increased risk” of the blaze spreading because the engine fire bottles had not been used.

Seerndv, и?

Seerndv,

I think sometimes [customers] want to hold our feet to the fire to make sure that we really do what we say we’re going to do, and I think some of it is a bit of grandstanding. I’m not going to have that debate with my customers.”
Я тогда не знаю зачем Flightglobal "обновил" тему.
Получил релиз репорта и обновил, нет ничего необычного. Пратт и Авиавласти наверняка договориль, когда можно информировать общественность, это уже коммерческая сторона вопроса.

Bombardier Inc’s CSeries showing ‘much higher’ reliability than competitors, Swiss Air Lines says
The first airline to fly Bombardier Inc.’s CSeries says the jetliner has been significantly more reliable in its first month of operation than other aircraft built by its competitors.

An aircraft’s dependability is often measured by its dispatch reliability, or the percentage of flights that depart on time. Swiss International Air Lines Ltd., which offered the first-ever CSeries commercial flight on July 15, said the aircraft is performing better than some Boeing Co. and Airbus Group SE jets did during their first few weeks in operation.

“The dispatch reliability of the first aircraft has already reached a high value, much higher than in comparison with the dispatch reliability of other new aircraft types during phase-in,” Swiss spokeswoman Karin Muller said in an email, pointing specifically to Airbus’s A380 and A320neo and Boeing’s 787.
She added that the CSeries’ dispatch reliability has not yet reached the desired level of 99 per cent, but Swiss won’t release more detailed numbers until at least five aircraft have been in operation for at least three months.
Swiss now has two CS100 aircraft in operation after the second one entered service last week. The airline has placed a firm order for 15 CS100s and 15 of the larger-variety CS300s.

The CSeries experienced some minor glitches in its first week of operation. The very first flight, from Zurich to Paris, was delayed by an hour when airport ramp staff disconnected a ground power cable too soon, forcing a power restart.

Four days later, a flight from Zurich to Manchester, U.K., turned back after the plane experienced a problem with its air-conditioning system.

Muller described these as “small technical and operational issues” that didn’t hurt the phase-in process.

“With our daily experience and the support by Bombardier on-site, these issues and insecurities regarding the handling have already noticeably decreased,” she said.

The feedback from our pilots is gratifying. They especially like the intuitive flying experience.

Swiss has also experienced some minor cabin design issues that it is addressing with Bombardier and which may delay delivery of the next aircraft by “a few weeks,” but overall the experience has been encouraging, Muller added.

“The customer feedback is very positive with the expected remarks concerning the bright cabin, reduced noise, enough leg room and space for hand luggage as well as the comfortable seats,” she said. “Also the feedback from our pilots is gratifying. They especially like the intuitive flying experience.”

Earlier this month, Bombardier CEO Alain Bellemare described the feedback from Swiss and its passengers as “outstanding.”

The positive reception has been a relief for the company, which had struggled with delays, cost overruns and serious doubts about demand for the aircraft. Recent orders from Air Canada and Delta Air Lines Inc. have helped to ease those concerns.

The larger CS300 version of the jetliner was certified by Transport Canada in July and is expected to enter service with Air Baltic Corp. by the end of the year.
